What is the Church's ruling on living wills?
Back in the 1970s, if a woman's marriage was annuled, did she have to take back her maiden name?

Questions on living wills are not matters of canon law but civil law related to morality.
There is no canon law provisions regarding the use of maiden names, although I would note that sacraments generally speaking are always recorded with the use of maiden names.
